Health Prescreen 

During your exercise sessions every effort will be made to assure your safety. However as with any exercise there are risks, including but not limited to, increased heart stress and the chance of injury. By agreeing to voluntarily engage in an acceptable plan of fitness training with Fitfusion, you agree to hold harmless, waive and release Fitfusion from any responsibility, claims, or demands arising from participating in sessions. You will be given exact instructions regarding the amount and kind of exercises you should do. A professionally trained fitness instructor will provide leadership to direct your activities, monitor your performance, and otherwise evaluate your effort. Depending upon your health status, you may or may not be required to have your heart rate aevaluated during these sessions to regulate you exercise within desired limits. By Agreeing, you understand that you are expected to follow staff instructions with regards to exercise. If you are taking prescribed medications, you have already so informed the trainer and further agree to so inform them promptly of any changes which your doctor or you have made with regards to its use. You further understand that during the performance of your training program, physical touching and positioning of your body may be necessary to assess your muscular and bodily reactions to specific exercises, as well as to ensure that you are using proper technique. You expressly consent to the physical contact for the stated reasons above also.

It is my understanding that I have been informed that there exists the remote possibility during exercise of adverse changes including, but not limited to, injuries, abnormal blood pressure, fainting, dizziness, disorders of heart rhythm, and in very rare instances heart attack, stroke, or even death. It is my desire to participate as herein indicated. I have read this informed consent form and fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily, without inducement.

BOOTCAMP MEMBERSHIP CANCELLATION/ON HOLD TERMS: 

If you wish to exit from our training package before the minimum term is completed, there will be no refund for sessions that have already been used and the minimum amount (balance agreement) must be paid in full.  However the remaining term (including the cost of attendance) may be transferred to a third party.

If you are outside of the first Month or Challenge term, on a 6month contract, a 35% upfront payment of the remaining amount applies if you wish to cancel your membership in the case where you can’t sell it to someone else.

By submitting the above forms you agree to honour and fulfil the terms and conditions of this agreement as set out below including but not restricted to:

  • You understand that once the minimum term has been fulfilled your membership payments will continue on a Weekly basis unless you inform FitFusion to cancel your membership. A 7 day notice of cancellation is required.

  • You agree that non-use of the facilities or services at FitFusion does not mean non-payment or a refund.

 

Membership ‘on hold’ period:​

  • Every 12 weeks you get up to 2 weeks where you can place your membership on hold. A minimum of 7 days notice is required for payments to go on hold.

Personal Training T's & C's 

(All weekly Personal Training clients have access to bonus unlimited bootcamps)

To ensure the continued delivery of high-quality sessions and the smooth operation of our services, the following terms and conditions apply to all personal training memberships at FitFusion
 

1. Membership Commitment

  • All personal training memberships require a minimum commitment of 6 weeks.

  • At the end of each 6-week period, you may request a hold of up to 1 week. Please note that hold weeks cannot be accumulated or carried over.

    • Holds or membership cancellations must be requested with a minimum of 1 week’s notice.

    • Any holds longer than 1 week will incur a base weekly fee of $39.50, which grants access to unlimited bootcamps or the option to receive a home/hotel workout plan.

  • If holds longer than 1 week are required, your training slot may be released to a waitlisted client unless the base rate fee is applied.
     

2. Session Cancellations

  • Minimum 6 hours' notice is required to cancel or reschedule a session.

  • Cancellations made with at least 6 hours’ notice will be charged:
    1 Session/week =  base weekly rate of $39.50 
    2 Sessions/week (both cancelled) = $49.50   (If you attend one session out of your two for the week and cancel the second with sufficient notice, a maximum charge of $25 will apply for the missed session.

  • Late cancellations (less than 6 hours’ notice) or no-shows will be charged at 100% of the session rate.

Note:  Whenever possible, all attempts will be made to reschedule your session. Alternatively, you will have access to bootcamps, home workout plans, and gym time.
 

3. Partner Training Sessions

  • For 2-person sessions, the full session rate still applies, even if one participant cannot attend.

  • We encourage clear and early communication between training partners to avoid misunderstandings.

4. Why These Terms Matter (Personal note from Tanya)

At FitFusion, I’m committed to providing you with the highest level of personalized care and attention. However, frequent last-minute cancellations and extended hold periods makes it challenging to maintain the consistency and quality of service that you deserve.

In the past late cancellations and prolonged hold periods often left me waiting at the gym for that unused hour—especially with another client booked directly afterward. On top of that, I wasn’t compensated for that time, which led to unpredictable and unsustainable finances, particularly when multiple clients canceled in one week.

While I always aim to be flexible and understanding, the updates are there to keep FitFusion running smoothly . These new terms are designed to create stability—for both of us—so I can continue offering the service, attention, and care you’ve come to expect.  This structure ensures that when you miss a session you continue to have access to

1.)  unlimited bootcamps,

2.)  use of the gym during staffed hours on your own time for that (1hour) missed session (needs to be arranged with your trainer)

3.) The option to receive a home workout

Additionally, these updates help ensure that your trainer is fairly compensated for the time reserved for your session and the work put into your program planning.
